So, you get married, changed your name, and are ready for your honeymoon abroad! But your passport reflects your new name? If you have legally changed your name, you need to revise either your passport or applied for a passport renewal. See below for Details.

  1. If your Name has been legally changed by marriage or by court order, you must inform the date, the issued your passport. If your passport was issued less than a year ago, you can apply for a change of name passport. Fee for this Service.
    • Print and fill out the US Passport Re, DS-5504. Provided that your Computer has Adobe Acrobat Reader, you can download the PDF form here.
    • Together with the completed form DS-5504, you need 2 passport size (2 inch x 2 inch) send a recent color photos, a certified copy of the marriage certificate or your name change court decree, and your current passport.
    • When using USPS, you send all the above to the following address:

    • If you supply a different service to use, send it to the following address:

      ATTN: Department 13290

      New castle, DE 19720
    • you will receive your passport within 6 weeks after the date of submission. If you use expedited Service, you will receive within 2 weeks after the date of the application for your passport.
  2. If your Name has changed due to a reason other than marriage or court order and/or your current passport is more than a year old, then you need to apply for a passport renewal and pay the renewal fees.
